If you would like to refer a patient, you may complete the online referral form or you may send referrals via email or fax (425-300-9057). Please include pertinent progress notes, diagnostic test results, imaging, photos, etc. prior to the patient’s appointment to maximize our consultation time.
When your referral is received, we will contact the patient to provide additional information about our clinic as well as a comprehensive intake form.
